Ordinary wired earphones use a standard wire straight to a speaker in your ear — safer than Bluetooth, but the wire itself can still act as an antenna. Air tube earphones go a step further: a shielded wire runs to a hollow tube rather than a speaker, so there’s no antenna-like wire near your ear and no speaker sitting inside it, keeping any EMF source as far from your head as the design allows.
